    Congenital Heart Disease, Vol.19, No.3, pp. 267-273, 2024, DOI:10.32604/chd.2024.051642 - 26 July 2024

Abstract The aorto-ventricular tunnel is a rare congenital cardiac anomaly. We present a case of aorto-ventricular tunnel diagnosed via fetal echocardiography. Emergency surgery was performed on the 2nd day of life to close the tunnel, located just in front of the right coronary ostium, due to the patient’s unstable health condition. The postoperative period revealed complete occlusion of the right coronary artery. Due to the patient’s stability, we opted not to reintervene on the right coronary artery. The patient fully recovered without the need for further coronary intervention. In cases of patients with an aorto-ventricular tunnel More >

Stubborn Hypoxia in Neonates with D-Transposition of the Great Arteries after Arterial Switch Operation: Central Sleep Apnea as the Cause and Potential Indicator of Brain Immaturity

    Camden L. Hebson1,*, Kyle Bliton2, Amr Y. Hammouda1, Kaitlyn Barr3, W. Hampton Gray4, Mohini Gunnett2, Waldemar F. Carlo1

    Congenital Heart Disease, Vol.19, No.2, pp. 185-195, 2024, DOI:10.32604/chd.2024.048871 - 16 May 2024

Abstract D-transposition of the great arteries (d-TGA) is surgically repaired with the arterial switch operation (ASO) with excellent results, however short and long-term morbidities still develop including neurocognitive delay. Clinically significant central sleep apnea is uncommon in non-premature infants, but when present indicates immature autonomic control of respiration likely due to a neurologic disorder. We report the unanticipated finding of central sleep apnea in four-term neonates with d-TGA after uncomplicated ASO, with the short-term complication of delayed hospital discharge and long-term concerns regarding this early marker of brain immaturity and its hindrance to normal development. Congenital Heart Disease, Vol.19, No.1, pp. 69-83, 2024, DOI:10.32604/chd.2024.045137 - 20 March 2024

Abstract Background: The optimal surgical timing and clinical outcomes of ventricular septal defect (VSD) closure in neonates remain unclear. We aimed to evaluate the clinical outcomes of VSD closure in neonates (age ≤ 30 days). Methods: We retrospectively reviewed 50 consecutive neonates who underwent VSD closure for isolated VSDs between August 2003 and June 2021. Indications for the procedure included congestive heart failure/failure to thrive and pulmonary hypertension. Major adverse events (MAEs) were defined as the composite of all-cause mortality, reoperation, persistent atrioventricular block, and significant (≥grade 2) valvular dysfunction. Results: The median age and body weight… More >

Electroencephalography (EEG) Based Neonatal Sleep Staging and Detection Using Various Classification Algorithms

Hafza Ayesha Siddiqa1, Muhammad Irfan1, Saadullah Farooq Abbasi2,*, Wei Chen1

    CMC-Computers, Materials & Continua, Vol.77, No.2, pp. 1759-1778, 2023, DOI:10.32604/cmc.2023.041970 - 29 November 2023

Abstract Automatic sleep staging of neonates is essential for monitoring their brain development and maturity of the nervous system. EEG based neonatal sleep staging provides valuable information about an infant’s growth and health, but is challenging due to the unique characteristics of EEG and lack of standardized protocols. This study aims to develop and compare 18 machine learning models using Automated Machine Learning (autoML) technique for accurate and reliable multi-channel EEG-based neonatal sleep-wake classification. The study investigates autoML feasibility without extensive manual selection of features or hyperparameter tuning. The data is obtained from neonates at post-menstrual… More >

Abstract Objectives: Over the last decade, neonatal repair has been advocated for many congenital heart diseases. However, specific subgroups of complex congenital heart disease still require temporary palliation for which both surgical and endovascular techniques are currently available. We reviewed our institutional experience with shunt palliation with an emphasis on risk factors for early mortality. Methods: This is a single-center retrospective study on 175 patients undergoing surgery for central shunt or modified Blalock-Taussig shunt. All data were extracted from a prospectively collected computerized database. We identified risk factors for early mortality by uni- and multi-variable analysis.… More >

Abstract Objective: Despite increasing enthusiasm for neonatal repair, patients with ductal-dependent circulation (pulmonary/systemic) or restrictive pulmonary blood flow still require initial palliation. Ductal stenting has emerged as an endovascular approach whereas modified-Blalock-Taussig and central shunt remain surgical references. In this study, we analyzed the relationship between pulmonary artery growth, sites of shunt connection, or antegrade pulmonary blood flow in surgically placed shunts. The need for secondary catheter-based interventions or pulmonary arterioplasty was also investigated. Methods: A retrospective single-center study analyzing 175 patients undergoing surgery for a central or modified-Blalock-Taussig shunt. Outcome growth variables were right pulmonary… More > Graphic Abstract

Abstract Background: Infants undergoing cardiac surgery with cardiopulmonary bypass (CPB) frequently receive intra-operative methylprednisolone (MP) to suppress CPB-related inflammation; however, the optimal dosing strategy and efficacy of MP remain unclear. Methods: We retrospectively analyzed all infants under 90 days-old who received intra-operative MP for cardiac surgery with CPB from 2014–2017 at our institution. We combined real-world dosing data from the electronic health record (EHR) and two previously developed population pharmacokinetic/pharmacodynamic models to simulate peak concentration (Cmax) and area under the concentration-time curve for 24 h (AUC24) for MP and the inflammatory cytokines interleukin-6 (IL-6) and interleukin-10… More > Graphic Abstract

Abstract Background: Most outcome studies in congenital cardiac surgery for “low weight” neonates include patients undergoing surgery without cardiopulmonary bypass (CPB). The primary objective of our study was to identify risk factors for in-hospital mortality in neonates weighing less than 3 Kg and undergoing surgery with CPB. In addition, we compared the effect of early surgery with CPB (before 37W-gestational age (GA)) for congenital heart disease to delayed surgery until a corrected GA of 37 weeks in an attempt to promote weight gain. Methods: Retrospective single-center study including all patients operated between 1997 and 2017. Uni- and… More >

Preoperative Feeding in Single Ventricle Neonates is Predictive of Shorter Time to Goal Feed

    Alyssia Venna1, Kathleen Reid2, Sarah Davis2, Jiaxiang Gai3, Yves d’Udekem1, Sarah Clauss2,*

    Congenital Heart Disease, Vol.17, No.5, pp. 505-518, 2022, DOI:10.32604/chd.2022.021571 - 06 September 2022

Abstract Background: Patients with single ventricle anatomy are at increased risk of growth failure and malnutrition. Amongst cardiac centers, there is little standardization of feeding practices in this complex population. We hypothesized that initiation of our center’s preoperative feeding protocol would result in decreased gastrostomy tube (G-tube) use, decreased length of stay and would not result in increased Necrotizing Enterocolitis (NEC) rates. Methods: A single institution review of 52 patients who had undergone stage I single ventricle palliative repair was performed. Patient diagnoses were hypoplastic left heart syndrome (39%), atrioventricular canal (15%), and other (46%). Postoperative parameters such… More > Graphic Abstract
