Abstract Introduction: We sought to investigate whether the development of sub-pulmonic systolic anterior motion (SAM) may be inherent to the anatomy of the the mitral valve (MV) or affected by external factors, such as a dilated right ventricle or chest abnormalities in d-looped transposition of the great arteries post atrial switch operation (d-TGA/AtS). Methods: Analysis was performed of clinical and cardiac imaging studies acquired on 19 adult patients with d-TGA/AtS (age 42 ± 6 years old, 56% male) between 2015–2019. Echocardiography data included mitral apparatus anatomy, and CT/MRI data included biventricular dimensions, function, and Haller index… More >

Characterization of Malalignment between Atrial and Ventricular Septa in Overriding/Straddling Tricuspid and Mitral Valves

    Mika Saito1,2, Dragulescu Andreea1,2, Shi-Joon Yoo1,2,3,*

    Congenital Heart Disease, Vol.16, No.1, pp. 1-16, 2021, DOI:10.32604/CHD.2021.011694 - 23 December 2020

Abstract Objective: Overriding/straddling of an atrioventricular valve is a consequence of atrial-ventricular septal malalignment. We sought to characterize the malalignment by analogizing it to a flap-door and using structured approach. Methods: The echocardiograms and magnetic resonance images of 35 patients with overriding/straddling tricuspid or mitral valve were evaluated to assess the following modifiers; the malaligned part of the septum, the reference structure, and the mechanism, direction and severity of malalignment. Results: The pathology included classic overriding tricuspid valve in 15, classic overriding mitral valve in 12, and overriding tricuspid valve with left juxtaposition of atrial appendages in 8.… More >

Angle of Attack Between Blood Flow and Mitral Valve Leaflets in Hypertrophic Obstructive Cardiomyopathy: An In Vivo Multi-patient CT-based FSI Study

Abstract The mechanisms of systolic anterior motion (SAM) of the mitral valve in hypertrophic obstructive cardiomyopathy (HOCM) remain unclear. To investigate the angle of attack between blood flow and mitral valve leaflets at pre-SAM time point, patient-specific CT-based computational models were constructed for 5 patients receiving septal myectomy surgery to obtain pre- and post-operative 2D vector flow mapping. The comparisons between pre- and post-operative angles of attack based on 2D vector flow mapping of 5 patients were performed. It was found that there was no statistically significant difference between pre- and post-operative angles of attack (61.1±t More >

Mitral valve prolapse and Marfan syndrome

    Amitabh Thacoor

    Congenital Heart Disease, Vol.12, No.4, pp. 430-434, 2017, DOI:10.1111/chd.12467

Abstract Marfan syndrome is a multisystemic genetic condition affecting connective tissue. It carries a reduced life expectancy, largely dependent on cardiovascular complications. More common cardiac manifestations such as aortic dissection and aortic valve incompetence have been widely documented in the literature. Mitral valve prolapse (MVP), however, has remained poorly documented. This article aims at exploring the existing literature on the pathophysiology and diagnosis of MVP in patients with Marfan syndrome, defining its current management and outlining the future developments surrounding it. NT-proBNP predicts mortality in adults with transposition of the great arteries late after Mustard or Senning correction

    Jana Rubáčková Popelová, MD1,2 | Markéta Tomková1,3 | Jakub Tomek1,4

    Congenital Heart Disease, Vol.12, No.4, pp. 448-457, 2017, DOI:10.1111/chd.12466

Abstract Objective: The patients after Mustard and Senning corrections of transposition of the great arteries (TGA) are at an increased risk of unexpected death. The aim of this study was to identify markers allowing risk stratification of patients after atrial switch correction of TGA to provide them with optimum care.
Methods and Results: In this study, 87 patients were retrospectively evaluated after atrial switch correction of TGA followed-up between 2005 and 2015. The mortality during the follow-up was 9% (8 cardiac deaths). Markers significantly predictive of death using univariable Cox proportional hazard ratio survival analysis were: N-terminal pro-B-type… More >

Tissue plasminogen activator for neonatal coronary thrombosis presenting with mitral valve regurgitation and impaired ventricular function

Delaram Molkara, Jose A. Silva Sepulveda, Thomas Do, Christopher Davis, Gregory P. Goldstein, John W. Moore, Howaida G El-Said

    Congenital Heart Disease, Vol.12, No.3, pp. 270-274, 2017, DOI:10.1111/chd.12432

Abstract Objective: Neonatal coronary thrombosis is a rarely reported disorder, with variable outcomes described. This study assessed the feasibility and safety of an institutional protocol using tissue plasminogen activator (tPA) administration for the treatment of neonatal coronary artery thrombi.
Methods: They reviewed the outcome of three neonates with clinical evidence of myocardial infarction secondary to coronary thrombosis. All three underwent the tPA treatment protocol.
Results: The three described cases presented at 5 hours, 15 hours, and 10 days of life. The patients identified underwent the tPA protocol at least once. There was clinical evidence of improvement in coronary flow, More >
